Clinical Characteristics of Femoral Hernia

Abstract
Purpose: Femoral hernia, a rare disease entity, is an exit of preperitoneal fat, urinary bladder, or peritoneal sac with/without contents through femoral canal. Mesh repair is limited for femoral hernia because incarceration or strangulation is frequent in femoral hernia. The objective of this study is to collect clinical data of femoral hernia and analyzed them for the purpose of decision making with proper repair.

Methods: Data of femoral hernia patients were retrospectively collected from electrical medical record system. Demographics, surgical details, and short-term outcomes of patients were collected.

Results: During the study period, a total of 1,709 groin hernia repairs were performed by a single surgeon, including 1,676 (98.1%) inguinal hernia repairs and 33 (1.9%) femoral hernia repairs. The 33 femoral patients included 22 (66.7%) female and 11 (33.3%) male femoral hernia patients. Femoral hernia patients were older than inguinal hernia patients [68.6¡¾12.1 vs. 56.5¡¾17.1 years, P£¼0.001]. Of the 33 femoral hernia patients, 21 (63.6%) were at the right side while 12 (36.4%) were at the left side. Thirty two (97.0%) of the 33 cases of femoral hernia were irreducible. Only one (3.0%) case was reducible. Recurrence occurred in 10 (30.3%) patients after femoral hernia repair. The recurrence was diagnosed immediate-postoperatively based on protruding mass in 5 cases. However, the recurrence time in the rest 5 patients could not be recognized.

Conclusion: Femoral hernia is dominant in females. Diagnosis of femoral hernia is difficult. The recurrent rate of femoral hernia is higher than that of inguinal hernia due to misdiagnosis. Therefore, even if the femoral hernia is asymptomatic, surgical repair should be recommended.
